Are you synchronizing from your internal network or from external.
If external, are you publishing Exchange RPC and Enterprise Vault with ISA Server ?

Internal only. 

The process I started yesterday on a test machine is still stuck at the same number of items and hasn't moved.  Still says synchronizing.

Yes, I am able to connect to that URL just fine.  I am able to search the vault from there without issue.

Not sure if this matters but USEINACTIVITYPERIOD is set to 1, would it help to change it to 0?

The folder option is empty and not in use.  Is that a problem?

Not sure, can you fill in a folder with enough free space (few gigabytes).
