BE14 not removing tape from slot after removing/cleaning/rebooting/scanning/inventory

There is a tape listed in Backup Exec 2014 and has been for about 2 weeks. Been rotating tapes but now this one is showing as just "there" even though the tape has been removed and stored offsite, a scan done, an inventory done, the BE services restarted, and backup server restarted.

I've also removed all tapes and it still shows as listed there. Done a clean job, still shows there. I've put a different tape in that same slot and it still shows as the old tape.

What would cause it to think a tape is in the drive?

Are you using a stand alone tape drive or a robotic library?  If so, exactly what brand & model?

Is that tape showing as being in a tape drive or in a robotic library slot?  your post mentions both so I am confused

If your issue is with a robotic library slot, is that slot configured to be a cleaning slot?  right click on the slot and check the properties.
