Backup Exec 12.0 SP3 with HP Library 1/8 920 - Adamm Mover Error: Read Failure!

Hi all!

From time to time I get unknown error:

Adamm Mover Error: Read Failure!
Error = ERROR_GEN_FAILURE
Drive = "HP 1"
{DB8EA6A0-B4A7-4DB5-9C3E-98F17C356939}
Media = "#034E50A6-001"
{00000000-0000-0000-0000-000000000000}
Read Mode: SingleBlock(0), ScsiPass(0)
Write Mode: SingleBlock(1), ScsiPass(1)

Source of error - Backup Exec, in system event get no errors.
AAlready tried to clean drive, change SCSI card, change SCSI cable, and so on, nothing help me.

When use hp ltt 4.8 try to write full tape with fake data, and ltt write it with no error.

In what direction to dig?

This is happening to me after BE 12.5 SP-3 was installed. This happend 6 times in a row around 4:50 AM EST and 5:10 AM EST. Although, it has not happend since. Not a lot of help but has your error gone away?

Symantec please let me/us know what to do.
