Guys the end result was the tape drive was knackered, I've now upgraded to SSR and use a NAS box.

1) Check if there are any hard read\write errors on the tape.

2) Perform cleaning of the Tape Drive

3) Update Symantec drivers for the Tape Drive.

4) Check Windows Event Viewer for any hardware related events like event id 7,9,11

I would start the SCSI tracer and see what command failed.

When did this start?  Has the tape drive been cleaned recently?
