Download the IBM tape utility and run the diagnostic checks against the drive to rule out hardware errors. You should also check the tape statistics and make sure there are no Hard Write Errors on the tapes which could cause issues with backups.
Thanks!

Check the adamm.log and the Event Viewer logs as well for any device-related errors.
What is the OS of the BE media server ? And are you using Symantec drivers for the tape drive ?

Windows Server 2008

Just 2008 or 2008 R2 ? If 2008 R2, then it is not supported with BE 12.x as media server.
If 2008, have you ran vendor diagnostics against the tape storage ?
Have you installed Symantec drivers for the tape drive and MS driver for the changer (if any) ?
Any hardware events logged in system log ?

Just Windows Server 2008 only.
Is there any diagnostics tool against the tape storgae? I'm using DELL power vault.
Below is the event log
Storage device "IBM 2" reported an error on a request to write data to media.
Error reported:
A tape read/write error has occurred. This is usually caused by dirty read/write heads in the tape drive. Clean the tape drive, and then try the job again. If the problem persists, try a different tape. You may also need to check for problems with cables, termination, or other hardware issues.

First, try using a cleaning tape to clean the drive.
If the error still crops up, choose the appropriate tool either from the vendor's site or from https://support.symantec.com/en_US/article.TECH176521.html
