Hi,
As a preliminary step ensure that the Device is listed in the Hardware Compatibility List.

Please perform following steps to resolve this issue :
1. Recreate tape device in Backup exec :
http://support.veritas.com/docs/192144
2. Create a new Backup job.
3. Repair database.
http://support.veritas.com/docs/265180
kindly also verify the following to resolve the issue.
- Ensure that the Drives are properly detected in Windows and are able to perform backup and restore using NTbackup.
- Ensure that SCSI ID 0 & 1 are not being used, as they are reserved for bootable devices. Also ensure that SCSI ID 6 is not being used.
- Disable removable storage (RSM) service on that device. In most of the cases, it has been seen that VERITAS device driver does not work properly with RSM if possible try to disable the service itself.
Ensure that you have the latest drivers installed
Refer the following Technote to install the Drivers.
http://seer.support.veritas.com/docs/199881.htm
We would also suggest, that you check system events for SCSI related events (7,9,11or 15) and in case you find any please refer to following technote/article for further information
http://support.microsoft.com/default.aspx?scid=kb;en-us;154690
If the above does not help kindly install the OEM Drivers and verify the results.
Refer the following Technote.
http://seer.support.veritas.com/docs/253974.htm
Alternatively run the backup job to a Backup-to-Disk folder and verify the results.
We hope this will help.
