Backup Fails - 0x45d, I/O device error

I have not had a successful backup on our new Windows 2003 standard server using Backup exec 9.1.

Tape device is an EXABYTE VXA-2
Tapes are all brand new. After the job fails the tape needs to be reformatted, renamed and re-added to the media set.

Updated veritas device drivers. Installed relevant Veritas updates. Also uninstalled and reinstalled Backup Exec.

The backup fails after around 15mins.

Error messages in the job log are:

Storage device "EXABYTE 1" reported an error on a request to write data to media.

Error reported:
The request could not be performed because of an I/O device error.

With reference to the IO errors which come up, we would like to mention you the following:- Generally, the IO errors are the first indication that a tape, SCSI controller, SCSI cable or tape device is going bad. It may also be indicative of outdated or incompatible firmware on the tape device itself.

Refer to the following steps to resolve this issue:

2) This error could occur due to SCSI configuration related issue. To resolve the issue, perform the following steps:

3) Also carry out backup job using NT backup utility ( Stop all the Backup Exec services before performing NT backup ).

I've tried all the above
- new tape drive
- separate controller to cdrom
- updated firmware
- reinstalled Veritas plus updates

Still same issues

Tapes are about 6months old.
