Hi,
Download the IBM tape tool utility. Stop the BE services, and run the diagnostics against the drive to rule out any hardware issues.
Also make sure you're using the Symantec drivers for the tape drive...run tapeinst.exe to do so. And if you're using Windows Server 2003 on which BE is installed, make sure the Removable Storage service is stopped and disabled...this causes issues with the application's use of the drive.
Thanks!

As previously advised, you should test your tape drive for hardware errors. However, you should use the diagnostic utility from the tape drive's manufacturer and not IBM's utility because you are using IBM tapes.
If the test shows errors, contact your tape drive vendor.
If the the drive passes the test, check that the tape drive is functioning correctly in the Windows Device Manager. If it is not functioning correctly, contact your tape drive vendor.
