The alerts display lots of "The driver detected a controller error on \Device\RaidPort1"

Event viewer displays Event ID: 33152 with the follow description

Error = ERROR_NOT_READY

Tape keeps popping out.

When I run in inventory, it fails.  I restart the Backup Exec services and then I am able to run an inventory.
Attempt to run the an inventory, it fails.  It's like it's not seeing the tape, even when this displays in Device Manager
Tried several other tapes and ran a cleaning tape.
New server, updated the firmware a few months back, appeared to work intermittently, but now it's not working at all.
 
Any suggestions?  Have been round in circiles on this one.

Is it connected to a HBA which is supporting RAID?  This configuration is not supported.

Correct, you must use a SCSI card, and not a SCSI RAID card.  This has been the case since the begining of time...
