Please see the error code info at https://vias.symantec.com/labs/vels/
(Enter V-16-2-13067 ).
This means that the resource monitor came back as offline when the resource State was online after the last monitor.

There might be output from the monitor or more info in the agent log.  Scroll up in the engine log for the last mention of the resource name, IND-DEL-CLU-003.

If the resource has it's own log, perhaps messages, or syslog.log, it might have logged something.

VCS is just cleaning up.

If this is a Application MonitorProgram, it's possible that the monitor program is too sensitive or easily fooled.

If data was logged in the engine, agent, or system logs, and/or if the agent is from Symantec, support can give you more information.

Looks like something happened outside to VCS...
