I have read through all the documentation for upgrading. Please see my concerns below.
Server 2008 R2 64bit with SQL Server 2005 on the same server. Should the SQL Dbase be on a separate system?
I should be able to do an in place upgrade by running the deployment scanner and applying the 10.0.3. cumulative hotfix?

You have two versions of SQL on the same server? Or you have two versions of SQL _and_ Enterprise Vault on the same system?
Is the EV system 32 bit? Or is it on Windows 2008 R2 x64 already?

Refer updated EV compatibility guide for version specific details of requirred compnents. http://www.symantec.com/docs/TECH38537
We always recommend to keep SQL & EV in different box altogather. If you current EV server is win 2k8 r2 -64 bit then you can perform upgrade to EV 10.0 SP3/SP4 from 9.0 SP X.
If you EV is currently is on 32 BIT OS then you can use migration wizard in order to migrate(& upgrade) to 10.0.x.

Here are the answers to your concerns:

Ans: First, you will need to move the SQL Server to seperate server before upgrading the EV. EV 10 requires atleast SQL 2005 SP3 or SP4. So make sure, when you prepare new Server for SQL, install SQL 2005 with SP3 or SP4. The Seperate SQL server is recommended for several performance reasons.

Ans: If I understand you correct, you have EV 9.0.2 running on Windows Server 2008 R2 x64 then you can perform in-place upgrade to EV 10.0.3.

If you do move your databases to a different SQL Server make sure Collations are identical otherwise......

As side-remark, be advised that the latest EV10 version is SP4. Might be best to go to that level.
