Hello,

You can re-apply the VSA password from the Enterprise Vault console (Directory properties). It will re-apply any permissions in DCOM components and Enterprise Vault services/resources.

Without a dtrace log, it's really difficult to determine what could cause that error, However, if you are running Enterprise Vault 10, you can setup a backtrace and use that event ID 4224 to trigger the dtrace:

How to run Backtrace to help diagnose issues with Enterprise Vault
http://www.symantec.com/docs/TECH180757

If you are running Enterprise Vault 9.0, you can setup a trigger in the dtrace:

How to run dtrace using triggers with Enterprise Vault (EV)
http://www.symantec.com/docs/TECH48505

I hope this helps.
