There is probably further error codes that are missing here. NetBackup will down a drive if it has trouble loading the media or if it gets I/O errors when the media is being read as well as a few other reasons.
I would like to see what happens if you mount and unmount a tape ouside of NetBackup using robtest.
Also please look at TECH31526 for Supplemental Information to Media Manager System Administrator's Guide for Automated Cartridge System Library Software. This will give you information to obtain logging.

Please do the following to help with troubleshooting:
Create bptm log folder under /usr/openv/netbackup/logs on master and all media servers sharing this drive.
Add VERBOSE entry in /usr/openv/volmgr/vm.conf on all servers sharing this drive and restart ltid. This will log additional Media manager events to /var/adm/messages.
Info in bptm logs and messages file will help to troubleshoot.
In the meantime, collect these files on all servers sharing this drive:
/usr/openv/netbackup/db/media/errors
Copy files to .txt files (e.g. media1-errors.txt, media2-errors.txt, master-errors.txt) and upload as File attachments.

On master server, run these commands to see which media server is holding reservation for this drive:
/usr/openv/volmgr/bin/vmdareq
and
/usr/openv/volmgr/bin/vmoprcmd

Think there was something about ACSLS and DISMOUNT DELAY, sorry have been a while since I worked with ACSLS so the exact setting note escapes me.
