Faulty drive? Anything logged in system messages or netbackup/db/media/errors file?

No drive available

Seems OS may have lost connectivity to device.
Please check output of 'tpconfig -l'

Find device name, then check OS status with this command:

mt -f /dev/rmt/#cbn status

For ongoing troubleshooting, add VERBOSE entry to /usr/openv/volmgr/vm.conf and restart NBU.
Media Manager errors will now be logged in /var/adm/messages.

Is that tape write protected?

You will have to try and release the reservation at OS level.

To release reservation, try the following:

mt -f  /dev/rmt/3cbn forcereserve
followed by:
mt -f  /dev/rmt/3cbn release

Keep an eye on /var/adm/messages to see if any I/O or hardware errors are seen for this drive.

Seems its not letting me to do:

 mt -f /dev/rmt/3cbn forcereserve
/dev/rmt/3cbn: write protected or reserved.
 mt -f /dev/rmt/3cbn release
/dev/rmt/3cbn: write protected or reserved.

You may have to go to the drive and physically power cycle it.

Please check status in ACSLS cmd_proc:

Hi Shekaib,

in case if its a windows server and If using McAfee antivirus , be sure to disable or follow http://www.symantec.com/docs/TECH56658 to make exceptions for Netbackup  processes.

Not sure but you can try  checking of rthe same
