it got an issue while reading the data on tape

tell us more info about your enviornment..

what is the OS and verions of media server, and also tape library / drive and tape details

also try to verify the images in media and see if the is giving any error  verify from GUI--> catalog --> verifry

you can also try duplication this image to different tape or disk stu to see if it is giving any errors..

If you have other drives, try downing drive HP.ULTRIUM5-SCSI.000 (index 0),  then retry the restore.

If it happens again using a different drive, you have problems with the tape or what's on the tape

This is not configuration issue with the backup software (NBU ). This is hardware error: TapeAlert Code: 0x05, Type: Critical, Flag: READ FAILURE, from drive HP.ULTRIUM5-SCSI.000 (index 0), Media Id A00023 NBU is merely reporting the hardware error. Check Event Viewer System log for hardware errors.

Last time was media ID: A00021

This time it's media ID: A00023

Questions:

1) Exactly which version of O/S is the media server running?

2) Which version of NetBackup is it running?

3) Have the media come from another evnvrionment, or another tape library?

4) Or were they written on the very same library that they you are trying to read on?

5) Do restores work from any other media?

6) What type/level/model are the media?  LTO?  If so, which of LTO1/2/3/4/5/6?

7) What type/level/model is the tape drive that you are trying to read the media on?

8) Is your tape drive stand-alone or in a tape library?

9) If you have a tape library - then do you have mixed tape drive type/level/model within the same library?

10) Are you able to post the relevant section from the related bptm log?  (or do you need some help with that?)

If you take the time to answer these questions, then I'm fairly sure that you'll get some help here.

However - I suspect that you/we will not be able to determine the true root cause until we see the relavent section of the bptm log - but don't worry, if you don't know how - then I'm sure someone may still help you.  (  ;)   In a way you have to help yourself and us a bit first, then we can help you more  ;)

FYI - Tape alert codes:

...and if you can get to the relevant area of the bptm logs, then these two links may help you understand the deeper codes:
