bpdbm consistency report returns: "open memory mapped file failed"

I have recently encountered such issues and following could be possible reasons:
1) RAM is not available to handle the request
2) BPTM cannot take new request and there is a chance that active backup might fail when the memory goes into low.

NBU version?
OS and version on master?
Master server specs?
