I've found a few topics on the subject on this forum, and some of them have links to the generic standards doc that defines the TapeAlerts. However, i found it to be, well, too generic. I finally found what I was looking for on IBM site - TapeAlert codes as implemented specifically on LTO drives. It describes the codes in pretty much plain English along with the actions to take. I think lot of people will find it useful. It can be found here:

Note: the codes are in decimal, you need to convert from hex as reported by NetBackup.
As an added bonus, there's also the breakdown of the error codes on the drive's LED panel. It can be found in Troubleshooting -> Resolving errors -> Resolving errors with the Ultrium tape drives
