netbackup 8 self service install issues

hello,

i am having some trouble here trying to install netbackup selfservice and i am stuck with dbase user id credentials, i have posted the screen shots, the last 2 i dont know what to enter, what ever user id i enter, it fails with error in the last screen. i have used ids' that exist on the server and ones that dont, but no id is accepted.

If I may ask - have you tried to find answers in the Installation Guide?

Have you verified that a Microsoft SQL Server 2012 or 2014 database is available?

Just came across this post, I appreciate it was a while ago so i hope you sorted out your problem.
