I have this problem.
When I Try to restore a file with netbackup 5.0 MP2 not in a original location but in a different location, I look this error:

the restore failed to recover the requested files(5)

I have create directory: C:\Program Files\VERITAS\NetBackup\db\altnames and insert No.Restrictions file

I have veritas netbackup 5.0 MP2 with win 2000 server SP4.The file that i try to restore is a simple jpg file (pics.jpg)

the error message looks interesting
can't create file: \\smith\d$\restore\pics.jpg

\\smith ??
I dont think severname should be in the path.
set source client to the actual client that backup was done on.
set destination to actual destination client for the restore

\d$ ??
This should be a physical not logical device
I would expect the full pathname to be something like:
D:\restore\pics.jpg

smith is a name of one client that I type in field: 'restore everything to a different location (maintening existing structure'.
I type: \\smith\d$\restore
But I believe, after your reply, that charather $ is not correct. I have tryed restored a file in a shared folder (always same client 'smith') and the restore go correctly.

is it netware?

I've seen restores fail as the logs do not exist on the media server when redirecting restores. Also is it complaining of UTF errors??

Andrew was able to get the restore done and the post is marked as answered.
It appears it was the use of $ in the pathname.
