Getting below error.
Too many data blocks written, check tape/driver block size configuration.

This error came from difference of tape position info that NetBackup expected from writing data size and NetBackup got from tape drive.
It is hard to point out what's wrong. It may caused by tape drive error, firmware defect, bug in OS or NetBackup. Check OS log if any relevant message is logged. Also consoder to upgrade NetBackup to 7.x. NetBackup 6.5 is EOSL version.

You can see more information in bptm logs and as yes this error can be caused because of Defective media, wrong media type,Incorrect drive configuration. You can verify the drive configuration again.

Also as Yasuhisa pointed that 6.x is already EOSL version. Please try and upgrade NetBackup as soon as possible.
