Header-Based Authentication

The SysAdmin guide is very light on details around header-based authentication. When I enable HBA, I receive the error message on the login page "You are not an authorized Clearwell user. Please contact your Clearwell administrator for assistance." with a Sign In button that does nothing. Any idea?
