Can't comment on other steps as but these ones look in a wrong order to me:
- newfs /dev/rdsk/c1t1d0s2
- vxdisksetup -i c1t1d0 format=sliced
vxdisksetup overwrites the label to take disk under vxvm control
once you init the disk, can you please post outputs for:
vxprint -eo alldgs list
and vxprint -g rootdg -hrt
