In my case, we use an ftp proxy and require the following the parameters to be passed in the session.

USER %u@%h %s
PASS %p
ACCT %w

or in a more readable format

USER FTP-Username <at> FTP-Server Proxy-User
PASS FTP-Password
ACCT Proxy-Password
