e0008234 Bad Cleaning Media

Hope that I post in the right category.

I've been searching from the Forum since last week but still can't find any solution.
Is there anyone can help for the above problem?

We are using Backup Exec 2010 since year ago, and this is the fourth time I hit this error message. This error has caused our backup stop running properly.
The last three errors were solved by changing new cleaning tape, but I don't really understand why it keeps giving me this error.

I've tried to remove the cleaning media from the Device and reset the number of mount, both don't help at all.
Have checked from server's Device Manager, and the device appeared as Unknown Medium Changer. Our server is on Ms Win Server 2008 R2.
Also, have tried to Initial Library and Inventory Device, both not working.

Is there any other way instead of changing new tape again? Would deeply appreciate for any suggestions.
