    What error do you get in the BE Gui when you try to create the dedupe disk?
    At what exact stage does that error appear? Is it straight after the summary screen?
    Does the dedupe disk appear red in the background at all or does it not get that far?
    Any errors / useful messages in the adamm.log file? (in the ..Backup Exec\Logs directory)

    The SGMon log above doesn't give much away. I collected a similar log whilst successfully creating a dedupe disk on a test machine and the SGMon looks much the same as yours above, except for the last couple of lines. Did you give it an unusual name, as from the SGMon log above it appears empty?
