Custom Reports Problem in BE 12.5

Good Morning,
My name is Pete and and I work for a managed services company. I have recently been asked to support our backup/recovery team. As we are shortly moving to Netbackup / Puredisk i am trying to get all the policy info out of BE 12.5 to make sure we have a record of what needs to be inplace on Netbackup. When I generate a policy name report or use custom reports to get the information i seem to get numerous duplicates of policy names. Please can someone advise me where I am going wrong.
