Error: e0000f16 Operation failed

I am getting this error: e0000f16 - The operation failed to acquire the minimum number of drives and media neeeded.

If I format the tape (I've used 2 different ones, one of which is really new) I can backup ok for about two or three jobs, but then I get the above error.  Once I get the error I have to reformat the tape before I can get the backup to work.  If not, I continue to get the error above without backing up a single byte.

Can someone help?
