Good day,

I have encounted this error while backing up to tape drive. Running on OS windows storage server 2003 sp2, backup exec 12.5 and LTO4 actidata tape drive.

Final error: 0xe00084c7 - A tape read/write error has occurred. This is usually caused by dirty read/write heads in the tape drive. Clean the tape drive, and then try the job again. If the problem persists, try a different tape. You may also need to check for problems with cables, termination, or other hardware issues. Final error category: Backup Media Errors

Tried to do cleaning on tape (work), insert cleaning tape fews time, update backup exec. But the problem still persist.

Any idea?

Since you have cleaned it, then it is most probably a hardware fault.  If you have a diagnostic utility, run it against the drive and make sure that you specify the write test.

Check the event viewer and look for system log with event 7,9,11,15. These are hardware related and shows when the hardware is corrupt. Also try to put new tapes and see if it works because this error comes when the tape drive header is corrupt or tape itself. If it is still the issue after putting the new tape then check all the scsci cableas and make sure it is connected. If all is well then problem is drive and needs replacement.
