tape keeps popping out

We have been using the Veritas backup exec for windows servers and once in a while the backup tape pops out, what does this mean?

Under what circumstances is the tape 'popping' out? Is it during a backup?
Please check if you have any alerts in the alerts tab when this happens.
Describe the device you are using and the tape capacity, size of the backup being executed.
