What licensing module is required for Sap on Oracle & Sap on SQL

For Oracle, it all depends on how you want to back up. If you want to use 'br-' commands and tools, you need NBU for SAP agent per tiered host.

NetBackup for SAP license supports Oracle,  SAPDB and MaxDB databases.

SAP for SQL needs the SQL agent. This is covered by Application and Database Pack license.
Also licensed per tiered host.
