Phase 2 metadata migration incomplete

During the upgrade from 7.1.0.4, we directly applied patch 7.5.0.5 followed by the Hotfixes. We started the services, and policies, and the backup processes already running.

But at the next day, we found out that the catalog cannot be browsed with error
 

"INF - unexpected return value from db_IMAGEreceive: unable to process request 228".

Then after investigating, we found out that phase 2 of metadata migration still not complete :

bash-4.2# /usr/openv/netbackup/bin/admincmd/nbemmcmd -listsettings -brief -machinename bckpapp1 |grep IMAGE
LIST_FS_IMAGE_HEADERS="1"

We already try to restart the image cleanup, but the status is still "1", with these messages :

There are skipped images.

How to solve this and how to finish the phase 2 metadata migration?

Hopefully you have bpdbm logging enabled? 

We need logs to find out what the problem is...

Here's the log. I see some SQL error there with VALUES ( ?, ? , ? .....)

I would log a call with Symantec Support right away.

We are not going to get it resolved here...
