Does anyone have solution why Stand alone drive offline and bad media?

The following issues have just started to appear,BAD MEDIA AND Drive OFFLINE? We are using VXA 1 tape backup and have updated from 9.0 to 9.1 as well as drivers and service pk4 we thought a issue was tape drive so we replaced the unit with another vxa 1 and now we are getting the stand alone device offline and bad media. In device mgr it shows unit working properly? Any suggestions...
