Duplicate Media Entries

Running BE 10.0 on SBS 2003 with a 10 tape library. Tapes are labeled with bar codes. When I try to restore, BE keeps asking me to insert a tape that is already in the library. In the 'Media' list most tapes are listed multiple times with different dates. I'm guessing BE actually thinks there are, for example, 3 different tapes with the label 'D000004'. When in reality there is only one. What causes this and how can I fix it? Thanks in advance.

I have the same problem running BE 10.1 SP2 on a Windows 2000 server but have disabled bar code rules in an attempt to fix this. What else can I try?

I've installed the latest hotfixes and I'm still having this problem. (It screws up the allocation times for every tape after this happens!) Please respond ASAP!
