Please install the latest Veritas Device Drivers of Backup Exec 9.1 from the following link :

Also check if the device is compatible with Veritas Backup Exec 9.1 and also listed in the Hardware Compatiblity List of HCL :

If the device is not supported then install the OEM (Original Equipment Manufacturer) drivers.

Also check the event logs (System and Application) logs for any errors. In system logs check for errors with Event Id's 7,9, or 11. They indicate hardware errors.

SCSI Id settings are also to be checked. The drive should always be connected to a SCSI Id more than that of the changer. SCSI Id's 0 and 1 are used to connect bootable devices.
