Possible that it could be a bad tape drive. Yet verify the following:
- Check that the device you use is tested by backup exec. Hers is the link for the Hardware compatibilty list:

- If it is compatible, ensure that you have installed the latest Veritas device drivers. If not, install them from the following link:

- upgrade the device firmware
- Take a backup on a backup-to-disk folder and test the results.

- as per the technote check if you get event id's 5,7,9 or 11 in the event viewer which indicate device related errors. In that case, you can contact the hardware manufacturer.
- try the job with a new tape.
