Unable to catalog

There might be some step I am leaving out but I have been unable to catalog media in my backup exec 2010 media server. 

The job failed with the following error: The requested media is not listed in the media index and could not be mounted. To add the media's catalog information to the disk-based catalogs, run an inventory operation on the media and resubmit the Catalog operation.

And yes I have donr the inventory first.
 

There might be some step I am leaving out but I have been unable to catalog media in my backup exec 2010 media server.  This tape originated on this server.

I have also attempted to restore tapes from an older server that was running 9 or 9.1.  In these cases the server has just rebooted after a period of time.
