I have exactly the same problem. My user is activated, and I updated from Office 2013 to Office 2016.
After updating the Outlook Plugin to the newest Version the TAB is still not showing.
What I tried so far:
- Remove and install again
- Check Regkeys
- Disable and re-enable the Plugin in Outlook

What verison of EV Add-in are you using? I'm running Office16 with EV 12(beta) and it is working. I have several users running O16 and EV11.1 and it's works as well. I had to run the ResetEVClient a couple times, but finally got it to work.
I am running 18.104.22.16874, which is working on one machine, but not on the other, which is quite strange.
Anyway, can you copy me the download link for the BETA? I would be happy to give it a try. I would only need the Outlook Add-in.
