EV8 toolbar for Entourage missing

I am current testing this toolbar. All went well until I had some kind of Entourage crash. When I restarted Entourage the toolbar did not come back.

I have tried reinstalling the toolbar, and rebooting the system. Nothing helped.

I can find nothing about this in the sparse documentation. I cannot even find where the application is installed so that I could try to start it manually. Does anyone know where this is situated?

What else can I do to revive this toolbar. There is certainly an icon for the app in "System Preferences" but this seems to be very basic and has the right setting (Entourage 2008). Any help appreciated.

Thanks. This was very helpful! It had an uninstall option - unistalling followed by reinstalling fixed the problem.
