Last Access Time vs. Last Modified

Work for a small business and using FSA on an 2003 File Server and just curious.  I setup one Policy for office files 6 months for Last Modified and got a huge number of files. I then setup another policy for office files based on last access time 6 months and got very little. Just curious anyone have experience with archiving based on last access time.

Don't forget there may be other processes for example Anti-Virus or even Backups that will change the last accessed date but will not modify the files.
