Disk to Tape Failing

I'm running Backup Exec 12.5 rev 2213 with the following hotfixes installed:

Im recieving the following error from my disk to tape backup on the verify portion. I've attached below the error and the job properties below.

Seems like a hardware issue...

Make sure the cabling to tape devices is proper.

Check event viewer to see if any events reporting tape hardware issues

Use OEM specific diagnostic utility to see if any problem with tape devices are reported.

Also ensure that the device drivers installed for the hardware is Symantec drivers as they are more compatible with BE software.

I looked on Symantec website to download the latest drivers, and I'm unable to locate them.
