Unable to backup Successfully

We have Veritas 9.1 installed on our SBS 2003 HP Server. For the past 3 months we have been unable to backup any job successfully. We have experienced various problems. One problem occurs in regards to backing up the exchange server mailboxs. Some mail boxs regiseter as corrupted and then next time they don't. Also when it reaches the point to start verification, it hangs saying "Loading Media" and continues no further. When successful backups do occur, we are able to inventory the tape, yet the catologue doesn't work. We have installed all updates but to no avail. Also tried reinstalling the product with no further success. All tapes are new and cleaning occuring regularly. WE are desparate for a solution as all attempts to rectify the problem have failed.

Any ideas on how to fix this issue
