I would expect nbdb upgrade error rather than database create error. Without any insight into the steps you followed or any installation logs, best to log a Severity 1 support call with Symantec / Veritas.

What is showing in your /usr/openv/db/log/server.log  and /usr/openv/netbackup/logs/nbdb/ ?

If it is something about assertion failure, you probably need a reinstallation follow by a catalog recovery - or call support.

Platform selected in opening post is W2003/2008. 

So, probably W2008?
