if you run tpautoconf -t in your media server, can you see the drive under the list? is the same path and serial number? if not then the issue is between your OS and your drive, can be an HBA, Zone or drive issue, if is correct, match the Drive Serial Number, Name and Path with the output of
vmoprcmd -autoconfig -t
vmoprcmd -shmdrive
vmglob -listall -b
if this dont match the drive will go down every time you try to use it.
