Duplicate items created in Virtual Vault

I have a user who is seeing duplicate items created in Virtual Vault (VV) when she moves an item from her VV Inbox to her VV 001 Year Retention folder.  So far, it appears this is the only user experiencing this and it does not seem to happen everytime she moves an item.  Any ideas?

Update:  This is happening to multiple users in the same vault store.

If you're able to reproduce it then I would suggest going through the configuration options in the Desktop Policy with Symantec Support.
