Configuring SAN client : Zoning best practices

Hi Team 

We have Netbackup appliance 5240 ( NBU version 3.0 ) , We have requirement to add around 15 physical servers as SAN client, so that the backups can run over the FC. 

I have few queries regarding cabling and zoning. 

a) Do you recommend using all four supported HBA ports ( 2 from each slot 5 & 6) for SAN client, or I can just connect one FC cable from fabric to one of the port and perform zoning.

b) Not sure, but if its a dual port HBA on these client, should I use these ports on the server to go to seperate ports on the appliance for multipathing, in case one HBA on the appliance goes down.

c) How many zones needs to be created... If client has single port HBA and we are utilizing just one single port on the appliance, would there be just one single zone for all of them, or there would be 15 zones for each target and initiator combination. 

Is there a more practical document ( apart from admin guide ) that I can share with the storage team.

Any help would be appreciated.
