Seems SCSI Persistent Reserve is enabled on your media server.
Try to disable it. Host Properties -> Media server -> Media
Select SPC-2 SCSI reserve option (NBU default).
Ensure bptm log folder exists on the media server and increase logging level for bptm to 3 (at least).
Post bptm log as File attachment if problem still exists after enabling SPC-2 SCSI reserve option.

What OS ?
SCSI reservation is the task of the operating system. Either the OS does not support SCSI persistent reservation or another host has already reserved the drive.
