Can you check the source to make sure that is meant to be an embedded attachment and not an image reference to a file that doesn't exist?
And by that I mean they could have linked to http://www.mySite.com/myPicture.jpg and it just happens to be myPicture.jpg no longer exists, so it's the same thing you'd see on a website when a resource no longer exists
Also what storage do you have for your vault store partitions? NTFS? Centera?

This looks like a retrieved message so I would be concerned if it was there before it was archived. Does this user have any non-standard Outlook add-ons? I came across an add-on years ago (don't remember the name anymore unfortunately!) which actually stripped the messages of attachments to a local spot on the computer and left a link to it. Although the message technically looked like it had an attachment, EV didn't. They noticed a discrepancy when it worked on one machine but didn't on another, which didn't have the local cache of attachments.
